3.1 The Role of the Patient Intake
The Raya Patient Intake Form is the foundation of care coordination. It:
- Establishes the patient's current clinical context
- Identifies medical, emotional, and social risks early
- Reduces repetitive questioning
- Drives Raya care pathways for mental health, SDOH, care coordination and condition management such as blood pressure, diabetes and nutrition
- Supports Medi-Cal / DHCS documentation requirements
It creates and continually modifies the patients care plan.
Patients are told they can skip questions, save and return, and that they will not be asked again unless something changes.
Doulas offer a variety of services, primarily focused on providing emotional, physical, and informational support during pregnancy, labor, birth, and postpartum.
