Visit Documentation Requirements
Documentation is how Raya:
- Coordinates care
- Supports payment
- Tracks outcomes
- Prevents disengagement
Requirements
Document every visit by completing the template based on the visit type. This takes less than 8 minutes per note. Complete free text sections when information is provided by the patient or by your assessment.
- Doulas are encouraged to complete notes within 24–48 hours of the visit.
- Use neutral, factual language
- No diagnoses or assumptions
- End every visit with a disposition
Delayed documentation may delay payment.
Documentation Submission Deadline
- All visit documentation must be submitted within four (4) months of the date the visit took place. These deadlines are set per our agreements with our health plans. Any documentation submitted more than four months after the visit date cannot be billed and is therefore not eligible for payment, even if the visit was completed and otherwise valid.
